My practice is an out of network practice which means that I do not contract directly with any insurance companies. Unfortunately, the insurance companies have too much say over how I am allowed to treat and I’ve found it in the best interest of my patients to be an out of network provider. Although I do not directly bill health insurance companies, I will, upon request, provide you with a treatment invoice of the services provided for you to submit for reimbursement.

I do not treat women’s or men’s pelvic health dysfunction, vestibular issues, or TMD. I also do not provide lymphedema management, cardiopulmonary therapy, workers compensation services, or neurological rehabilitation for things such as spinal cord injuries or strokes.

Come dressed in comfortable clothes that are easy to move in. For the first visit, it is helpful if you wear loose fitting clothing in order for me to be able to look at the injured area. Please also wear tennis shoes to ensure safety when in the gym area.
Intake forms should be filled out prior to your first appointment. If you have questions about any of the forms, feel free to wait until to talk to me to sign them.

I accept cash, credit/debit card(s), and HSA/FSA payments. I ask for a $50 deposit prior to your first appointment. This deposit holds your spot and will be credited to your visit payment.
